“The department as a whole had lost confidence in leadership (previous) following what happened,” he said. “And there were great things that have been identified as things that had to be corrected as quickly as we could repair them: things like intelligence failures, communication failures, equipment failures, training failures, leadership failures – all these things necessary to be treated immediately.”
