City are two points clear and will win a fourth successive title if they beat the Hammers.
The Gunners have a better goal difference, so a draw against City and a win against Everton would be enough.
There will be identical trophies at the Emirates and Etihad, with 40 medals up for grabs at each stadium.
Arsenal manager Mikel Arteta said of the last few days: “It’s one of the most important weeks for many of us, and it’s certainly the most important moment.
“I’m really excited and looking forward to Sunday, and obviously I’m also very optimistic about the possible outcome.”
Asked if he should relieve the players after City’s win over Tottenham on Tuesday, Arteta added: “The hope was there and it didn’t happen and now we have to do our job which is going to be difficult because Everton are in a difficult situation.” It’s a very good moment, so we will have to prove once again that we can be better than the opponent and win the match.
“Then we have to wish for the best and that West Ham have a really good day and help us realize our dream. That’s what we hope.”
