This is the first time that a vaccine has expired and is no longer in use by the Milwaukee Department of Health, the department said in a statement Tuesday.

The problem arose when medical staff were instructed not to give the Pfizer vaccine to teachers until that day, according to the health department. This was made worse when a number of teachers scheduled Monday appointments without identifying themselves as teachers until they arrived at the Wisconsin Center.

As the expiration date approached, health service workers rushed to locate vaccine-eligible people who had not yet received theirs. This effort reduced the number of potentially wasted doses from 140 to 34 that expired.

The incident is currently under review by the Milwaukee Department of Health. At present, the ministry says there is no evidence to suggest intentional or malicious action.

A total of 1,058 doses were administered at the Wisconsin Center on Monday. More than 25,000 doses have been administered by the health department since the start of vaccination efforts.
